Bit of a thread hijack going on here but I'll bite. In my experience people tend to use ClamAV on Linux mostly to defend against viruses getting on their Windows systems, for example on a mail gateway or web proxy. That way ClamAV can stop the viruses it knows about from ever getting to the Windows machines in the first place. ClamAV also picks up a few Linux malware as well, and I'm pretty sure I've seen an on-access scanning module that plugs into ClamAV as well, but I've never used that functionality.
