This is not any clearer as the first, you have to do a lot of thinking before finding out what may be meant, and the fact that there are two ballots only confuse that. I think that if you send a ballot where the response expected from the voter is yes or no, then at least you should include the question to which the user reply in the ballot, in a clear way, so as to not confuse people.
