It looks like multiple cycles of suspend/resume or X server upgrade without restarting X triggers this. I tried single suspend/resume before and it did not trigger the bug. Now the X server is ii xserver-xorg-core 1.2.99.903-1 X.Org X server -- core server I upgraded after an X lockup.
