The MIME/SMTP architects must have envisioned some use for two different
names or there would be no reason to specify it twice. Does one take
precedents over the other?

The short answer is that this is a known vulnerability, so it must be caught. :)

The slightly longer answer is that there is no known reason to specify the filename twice in two separate formats, and the RFCs do not specify which should be used.
