Did you change versions of McAfee? (looks like you stayed with 4, but just in case)
I recently did a new mail server here and changed from 4.5 on the old server to 7.1 on the new server (also from NT Server to Server 2003).
7.1 works fine, but it used a different directory.
To confirm you might want to copy/paste your DOS command line into a dos window to check the path.
Also you may have to test (like with Eicar) from outside of the mail server. My local tests (with Imail client) did not trigger a virus alert, but from my desk (Netscape did).
