Do a search for ExHook32 and ParseRIP. I have the website but it appears to
be a dead link. Basically this util creates a RIP file when an exception etc
occurs. You then load the RIP file along with your source file and it goes
to the place in the code that was last executed.
I don't know if it still in circulation, I have it for D3 but don't use it
that much. The main problem is it needs the MAP files and I can never
remember to turn them on when Compiling. I have never tried it will a Dll.
It is shareware though so it might be worth trying.

Subject: [DUG]: address and source code
Hi,
One of my DLL occasionally cause an access violation error (says at address
015D7B5C in module "MyDLL.DLL" )in my client system, but it is hard to
explain when and how this happens. So I find very hard to use debug tools in
Delphi to test against client's environment.
Anyone here knows any tool can generate a list which can show both the
Delphi source code and the address (or relative address) in Delphi linked
exe/DLL?
Jim
