GenHelpPro is what a lot of developers use. Many options and a reasonable
price.
Time2Help is also another one that is used (and a guy I know uses it for his
components and application source).
For my apps Help files I use HelpNDoc (doesn't do source).

I'm currently looking at Doc-O-Matic which seems to be the preferred choice,
but very pricey.
