Reduce the set of possible instance states and transitions

The set of possible instance states is still too big for clients to deal with
effectively. We need to reduce that set (possibly to only PENDING, RUNNING, and
TERMINATED) and the actions for each of them.
This also needs to be documented properly, including how native cloud states
are mapped to the restricted set of states for each driver.
This needs to be done across all drivers.
