1.
Comment out the SMTP_USERNAME and SMTP_PASSWORD in your config file.  They 
are only used if your SMTP server requires authentication.  Based on 
the error message you're getting, your SMTP server does not require it.

2.
Are you referring to /var/log/denyhosts or some other log?  Try running DH 
in debug mode and monitor the long to see what happens after the 
SYNC_INTERVAL is reached (you may want to lower it to 5m or so for 
testing).

3.
I need to fix that issue but you can safely ignore it since the string 
liekly doesn't contain a host there is nothing for DH to do with it (it 
needs a host in order to block it).
