I got a new error ...
Quoting Jeff Levitt:

On my machine, it was "Getting Started".
So, executing "ant" in the directory
trunk/DITA-OT1.0/
I fed it the ditamap filename
/opt/trunk/src/getstart/getstartderby.ditamap
and "pdf".
... resulting in a fatal error (in both JDK 1.4 and JDK5):
[xslt] IDXS015E Error: (File =
/opt/trunk/src/getstart/getstartderby.ditamap, Element = topicref:7)
[xslt] Unknown file extension in href:
[xslt] cgsinstallingderby.dita If this is a link to a
[xslt] non-DITA resource, set the format attribute to
match the
[xslt] resource (for example, 'txt', 'pdf', or 'html').
If it's a
[xslt] link to a DITA resource, the file extension must
be .dita .
