Thanks for the review Knut. I'll create another patch to address your comments.

Wrt. WriteChainAlreadyInUseError being caught: No I don't think we can 
guarantee that. And even if we analysed the code and concluded that it is safe, 
there is no guarantee that it will remain so if the code is modified later. My 
thoughts about this is that starting another writeChain without finishing the 
previous one, when you already have sent data to server, usually indicates a 
bug, in much the same way as an NPE or ArrayIndexOutOfBoundsException does. I'm 
fairly certain that whenever this situation occurs the result would be some 
kind of hard-to-understand syntax/parsing error from the server, and my 
reasoning was that even an uncaught WriteChainAlreadyInUseError was a 
preferable.

It is always possible to make it checked and let it be wrapped in SqlExceptions 
and IOExceptions on its way up the callstack, but then we need to traverse the 
cause-chain to trap the situation where you're setting a stream backed by your 
own connection.
