Thanks, Kim! I think the placement is fine, but it might be better to have it a
little earlier. For example, I think applications that suffer from deadlocks
should verify that they have the appropriate indexes before they start using
table locks, so maybe swap those two paragraphs? I liked the cross reference to
the Tuning Guide. It seems to contain all the useful details about what
"appropriate use of indexes" means, so that we can keep the Developer's Guide
topic short.
