Attaching patch (and resulting html output) to function as a compromise between 
most comments to this issue so far, with hopes that this issue can eventually 
be resolved and closed. Please review, and commit if all is well:

I have not seen any response to 
http://article.gmane.org/gmane.comp.apache.db.derby.devel/20833, so I have not 
included all the consistency-related suggestions in the patch.
