Thanks, but I can hardly take credit for it. It is just the format I use at work. Apparently these five words come from the collected experience of almost 100 years supporting users #:)

I'm thinking that maybe we should establish this Problem/Symptom/Cause/Solution/Workaround
format, maybe somewhere in the Wiki, as a sort of mini-standard, since it
seems to cover all the areas that I can think of for release notes.

I think that sounds great.

Do we already have such a page somewhere? Or should I go ahead and make one?
