thanks Oleksandr and Øystein for the feed back. In this new version I have
fixed the typos and elaborated sections related to introduction, test case and
documentation.
About the changes in unique index a new index will be required to support
unique constraints on null able columns but weather to expose the new type of
index to user is not clear.
Probably I should split this issue one about unique constraint and another to
expose the backing index directly to the users. I will take up the 2ns task too
if its decided that its good idea to have it.
