I think one issue that hasn't been tackled for a long time are the menu entries, which can at times be a bit confusing or are at least not very intuitive to use. I started adapting Writer's Insert menu to the way I use it. I grouped the items actually inserting some kind of object and separate from these the items dealing with text, like footers, indexes etc. At the bottom are menu entries dealing with some sort of embedding ( I assume that relatively few people use that compared to the top items. ) That way the distance between the 'Insert' menu button and the (in my case) most used items was decreased. Additionally I enabled the icons in menus, which IMHO makes it much easier to find a certain function. An advantage is, that this probably increases usability quite a bit and is very easy to 'implement'.
