Outlook (2003) shows the indicators only once each. "AM" is shown
with the first visible hour cell (it moves as you scroll), while "PM"
is shown at noon, where it first appears.

I like Chandler's darker grey calendar border for work hours easier to
see, though, and sufficient to indicate which part of the day I'm
looking at. It may be because the app window is taking up most of my
screen, so I always see the boundary between darker and lighter border.
That, and the fact that all my events tend to happen during those hours.
