Binary package hint: evince
Please take a look at the attached PDF file.
If you open it in evince, the "Yes" checkboxes in 11, 14 and 16 are
marked, and the "No" checkboxes are not marked.
If you open the same file in Acrobat Reader, the "No" checkboxes are
marked and the 'Yes' checkboxes are unmarked.
Something seems wrong here.
