Alex - here is what I noted from your data:

The Storage icon is the same as the Filesystem icon when mounted because
the Gnome icon theme does not contain a "drive-harddisk-usb" icon. If
the theme contains one, then it will be used. I have a theme with that
icon, and my USB drives have a different icon to the Filesystem icon
(the Human theme is also ok). If you don't like it, you should report it
to the Gnome bugzilla.

When an external drive is connected via USB but unmounted, it will show
the "drive-removable-media-usb" icon. For the Gnome icon theme, that is
a picture of a drive with a slot. The Human theme uses a picture of a
drive with a USB icon. The theme I use shows a picture of a thumb drive
for this icon. If you don't like it, you should report this to the Gnome
bugzilla.

As for the Filesystem icon, I'm not sure what other type of icon would
be suitable. I don't agree with you that this one is wrong. Can you make
a better suggestion?

Your Macintosh icon uses the "drive-removable-media" icon when not
mounted. This is because it is "removable" in the sense that it can be
mounted/unmounted by a normal user via HAL. What other icon do you
suggest when the filesystem has been "removed" (ie, unmounted) from the
system? This icon will use the "drive-harddisk" icon when mounted, which
is correct.

I'm going to reassign this to the gnome-icon-theme, as most of your
problems really are theme-specific. Please could you forward this
upstream to the Gnome bugzilla?

If you think that displaying "drive-removable-media" for unmounted
internal volumes is a bug, please also feel free to report that to the
bugzilla too.

Thanks
