Binary package hint: nautilus
Suppose you have two files in one directory, each one has a filename two
bytes long. The bytes of the first one (in hex) are 0xad34 and the
bytes of the second one (in hex) are 0xd334 . If your locale is utf-8,
then Nautilus will render these two files as:
<?>4 (invalid encoding)
and
<?>4 (invalid encoding)
So far so good. Now open another Nautilus window and drag and drop the
first file into there. It disappears from the first window and appears
in the second. Now you have two windows, each of which has one file
whose name is rendered as "<?>4 (invalid encoding)". Now drag and drop
the second file. It disappears from the first window but doesn't appear
in the second! Oh, horrors -- did I lose my file? No, it is there, you
just have to make the second window refresh to see it.
Uh-oh, I just tried this again while typing up this bug report and the
bug didn't happen this time! When I dragged and dropped the second
file, the target window refreshed. Hm. The first time I had a bigger,
more complicated colliding filename than just these two two-byte ones.
Oh, and when I right-click to add a folder into this window which
contains these two colliding filenames then the folder doesn't appear
until I force the window to refresh!
Whoops, now it does again. There is some hidden state here so that
sometimes the window doesn't refresh automatically and sometimes it
does. I don't know if it has to do with these colliding filenames or
not.

If you drag and drop a file whose name is invalidly encoded and whose mangled
name collides with an extant file then the window doesn't refresh.
