Same issue, AMD e-350 and 12.04 Precise. This is a pretty new system and I thought that it may be a hardware issue. This confirms that it is a driver issue.
MSI E350IA-E45 Motherboard The HDMI works fine for a while but randomly disappears. When the sound stops I can look in the sound settings and the HDMI option is gone. If I log out and log in again it will reappear and work fine for a while.
