Same problem in 12.04. This happened sometime after a gnome-control- center (and ....-data) update (to 1:3.4.2-0ubuntu0.13.1) this morning. I tried going back to the previous version (1:3.4.1-0ubuntu1), to no avail.
Both Alsa and PulseAudio sound still work, but the failure of the Test Sound facility is a real nuisance.
