1. Looks like preferences window is created as CSD window. This is not metacity bug - CSD windows are not decorated by metacity. This looks like problem also in Unity session... I would suggest to not use Ambiance theme with Flashback.
2. I can reproduce this in Ubuntu session, but I can not reproduce in my JHBuild session with metacity from current git master and gnome-3-18 branches... Might be ubuntu specific bug... 3. I can not fix this without clear steps how to reproduce.
