I've never heard of it happening by accident.

I suppose prior to pressing that key combo it might have been interesting to 
check your default GSettings for it:
   https://wiki.gnome.org/Projects/GnomeShell/Magnification

Now that you've toggled it, checking the GSettings would be
inconclusive. Perhaps it got stuck on somehow as a result of your system
upgrades (the bug description shows you last did a fresh install on
Ubuntu 12.10).
