At one point I tried to use the "draw button" code from the metacity theme engine to draw the menu icons, but it looked like ass for most themes. So I think you have it right, it may make sense if the metacity theme can somehow provide the menu icons, but I don't think we can do it automatically exactly. Maybe the right approach is the inverse, the metacity theme can render icons from the icon theme into the titlebar if it wants to.
