Happens to me to. I don't know how to investigate on this bug. Are there any log files related to appearance/themes? Once in every two or three weeks ubuntu falls back on a different theme.

I can't change the theme, because clicking on other themes only changes it for the window of the program "Appearance", other windows stay with the fallback theme. The icon theme will stay the fallback even in the Apprearance window. After I log out and back in, everything is fine. I don't know how to reproduce it yet. It happens like once in 2 weeks.
