I seem to identify the cause of the issue. As of Precise global
shortcuts began to depend on current layout, which is wrong.

Say, I assign the global shortcut Ctrl+Alt+Shift+D to run an
application. But if I have Russian layout selected in the current
application, the system translates it into Ctrl+Alt+Shift+"Cyrillic ve"
and ignores it.

Global shortcuts depending on current keyboard layout in the application
is a sort of nonsense, I suppose.

Bug description:
  After upgrade to Ubuntu 12.04 Precise some keyboard shortcuts assigned
  via System Settings -> Keyboard fail to work without any sensible
  reason. This applies to both standard (like Volume Up/Down) and custom
  shortcuts.

  I have a lot of custom shortcuts for starting applications, like
  Ctrl+Alt+Shift+<Letter>, half of them does work, half of them doesn't.
  Which ones exactly—changes after every logon or reassigning shortcuts
  in the Keyboard.
