This error happens when fall-back icon theme not installed or not choosed in .gtk2.config file. For instance, I have same issue with Gentoo Linux (KDE 4), where fallback icon theme was not present, so solution was to install package 'x11-themes/gnome-icon-theme' and apply it in KDE system configuration application.
