Thank you for your bug report, you list several issues which are each
duplicate bugs so I can't mark the bug as duplicate of one other ticket,
marking it as invalid instead, the  gimp issue is bug #995916 and a SRU
is being working with a fix, the keepass case has been mentioned in
other bugs (I don't find the numbers atm though)

Title:
  Some running app icons don't show up in the launcher

Status in “unity” package in Ubuntu:
  Invalid

Bug description:
  Some running applications don't have a launcher tile, nor do they show
  up in the Alt-tab list. The gimp does this for me all the time. When I
  launch keepassx, I get a tile for the password dialog, and then it
  goes away once the main window opens. I am able to switch to these
  applications with Super-W, but not alt-tab and not the launcher, of
  course.
