Alt-tab switcher freezing

Bug description:
This machine has been upgraded from Natty. This is happening every
single day for no reason when I alt tab through my applications. The
switcher I have attached will not go away. I cannot access any
application as it won't go into focus into an application. The only
way out of this is to reboot the machine.
To reproduce.
1 Open up a few applications
2 ALT Tab through the applications
3 sometimes it freezes but not always. so hard to reproduce
