  Icons and theme not appearing correctly

Status in “unity” package in Ubuntu:
  Expired

Bug description:
  I am using the radiance theme, and after upgrading quantal and
  rebooting my window title have the darker ambiance theme and so does
  the menu bar. In addition icons appear to look like the default GNOME
  type icons. Attached a screenshot.
