File -> New -> Labels -> tab Labels -> drop down Brand Avery Letter Size
-> drop down Type 5160 Address, then selected the database I was using,
and the table (I think it was something like 'Table 1'), added the
appropriate fields into the  label text, then clicked New Document.

Bug description:
  When a 3 column label sheet is chosen for printing addresses with
  mailmerge, columns 2 and 3 are aligned further and further to the
  left. On Avery 5160, the rightmost column (3) is aligned so far to the
  left that it does not print fully on the label. This has been reported
  as Debian Bug #678813 and LibreOffice #58700.
