Bug is still present in Ubuntu 12.10 / evince 3.6.0-0ubuntu2. Opening a document with forms, data filled in forms just won't show. It won't print either. Going in the form field (once you know there is one !) shows the data, until you leave the field.
Works just fine in Okular (okular ask if you want to show/hide the form).
