Desktop with Intel e8500 cpu, Nvidia GT9500 and default (non-Nvidia) video
driver.
I get this problem frequently following a restart from 12.04.
Pressing the reset button or executing a FULL power shutdown prior to
completion of booting 13.04 will usually eliminate this problem. This
would imply that a hardware state that is properly initialized following
a reset or full power shutdown is NOT being properly initialized during
a warm reboot.
On occasion, I have also had to update/install grub, either from 12.04
or a via fix a broken system from a server CD.

Bug description:
I'm not entirely sure that this is xorg, I can't figure out which log file to
look in to find out. The log files with likely names did not give me usefull
info.
I've seen similar errors, but never the same hardware or circumstanses.
Behavior started immediatly after upgrade to 12.10
I'm running GNOME classic (though I think this bug is occuring before
this becomes relevent), using an ATI HD4850 video card with open
source drivers.
About every other boot, I get a white message box telling me that something
went wrong and the system will boot in low graphics mode. I'll try and write
down the exact message next time this happens.
The first time this happend (the next day after upgrading to 12.10, I was
able to select the "use defaults" option to continue. every time after this,
the "use defaults" or "use latest backup" goes right back to the same question.
The only way forward is to boot into low graphics mode.
I usually shut down the computer at the login screen when it is in low
graphics mode (from the menu, not the switch), and turn the computer
back on. This has booted cleanly with the correct graphics mode every
time I have done it (about 4 times). The one time I tried "sudo
shutdown -r now", it rebooted and had the same problem again
