1) download
http://www.bhw.de/content/Service/FormularCenter/D_135_a_%28mL%29.pdf
2) open the file in evince
3) input some text in the field below "Vertragsnummer" and leave the field
The text displays at about half the width it should be (see attached
screenshot)
