Go to Tools -> Footnotes / Endnotes.  Change Start at: 2 to be Start at:
1

I totally missed that when I first checked the settings.  Closing
because it's not a bug.

  Footnote numbers start at 2 instead of 1

Status in “libreoffice” package in Ubuntu:
  Invalid

Bug description:
  Some how a document has gotten munged so that the footnotes start at 2
  instead of at 1.  Cutting and pasting the text from the document into
  a new, fresh document corrects the problem.  But apparently no amount
  of fiddling can get the count to start at 1 in the old document.
