Sorry, I have to amend my last comment.

If I open the attached file "Original.doc" with LO and save it again as
a doc file and afterwards reopen it with MSO 2007 and LO, then in both
programmes the heading is missing.  In this case I can confirm a missing
header with LO 4.0.2.2 (Win7 Home, 64bit).

  4) What happens instead is the header on page 2 is missing.
  Original.doc was created in MS Office.
