Are you still getting this?
DIalogs seems fine to me, but I see this problem with the "Database
Form", but this is actually a LO issue, since it matches the window with
"libreoffice-startcenter" class, while it should use "libreoffice-base"
instead.

With bug #1063862 fixed, bamf works now fine for the Base main
application, but the dialog windows started from there still show a
blank icon. See screenshot at
