I was able to simplify it this file and was still able to produce this and have included it here as well. LibreOffice document b.odt attached LibreOffice pdf printout document b.pdf, which is normal, attached Printer prints document almost correctly, however, the image as mentioned above is warped. I can scan this output and add it if so requested.
