Maybe I'm having the same problem: Since last firefox update (yesterday or so), the bookmarks' list icon (not the bookmarks' window one) disappeared.
While i'm editing custom settings for navigation, personnalized and modules bar, I see it, but when I'm done editing, it disappears. I discovered that it stays available if I place it in the modules bar (the one at the bottom). If I place it in any other bar, it vanishes.
