For me it looks like that your document fits only into the printable
area of A4.SM and that Okular is still using strange custom page sizes.
Can you print from both Okular and evince iiwth A4.SM again, check
whether your result (evince works, Okular cuts) gets reproduced and post
the error_log (CUPS in debug mode) for this?

Bug description:
  Here is the example pdf file attached and also two photos of the printed page.
  I used RTL because it demonstrates better the edge that is cut off - the 
right edge.

  This is supposed to be an A4 PDF (the file is attached so you can check).
  And this is supposed to be an A4 (210x297) printout (the printer settings are 
that).
