On Ubuntu 12.04 x64 using kernel 3.11.1-031101-generic (which supports
USB audio), I am able to get one of two USB audio devices working
(Turtlebeach AudioAdvantage Micro = CM102-A+/102S+ Audio Controller,
lsusb ID 0d8c:0103) .  This proves snd-usb-audio is loaded and working.

The device which is not working (reason for this bug report) is an AKAI
EIE Pro.  Per lsusb, it has  ID 09e8:0010.  However, when plugged in, it
does not show up with cat /proc/asound/cards .

The same USB audio devices operates fine on the same system under
Windows 7 x64, proving it's not a HW problem or a 32 vs 64 bit issue.
