I'll add some informations I think could be useful.

My conf : 
Kubuntu 13.10, KDE+Openbox (no compiz), Geforce 8400 GS, Nvidia driver 304.88.
Two screens : Primary : Idek Iiyama PL2206W on DVI, [email protected]
Secondary : Samsung SyncMaster on VGA, [email protected]

I can see clearly tearing in VLC. But what's interesting is that I can
see tearing in the primary monitor and not the secondary if the output
of VLC is set to GLX, and I can see tearing on the secondary monitor but
not the primary if the output is set to XVideo.

  Vsync not working with nvidia & dual monitors

Status in “nvidia-graphics-drivers” package in Ubuntu:
  Confirmed

Bug description:
  Using a twinview setup, vsync will not work on one of the monitors -
  the same one, regardless of which one the display is set to sync to.
  Both monitors have the same refresh rate - 60 Hz - and the same
  vertical resolution.

  Using the restricted 185 drivers, installed from the "hardware
  drivers" screen. Sync to vblank turned on in both nvidia-settings and
  compizconfig settings manager. The vsync problem also occurs if compiz
  is disabled entirely.

  This happened on a fresh install of Karmic alpha 5.
