Daniel Dickinson, this bug was reported a while ago and there hasn't
been any activity in it recently. We were wondering if this is still an
issue? If so, could you please test for this with the latest development
release of Ubuntu? ISO images are available from
http://cdimage.ubuntu.com/daily-live/current/ .

If it remains an issue, could you please run the following command in
the development release from a Terminal
(Applications->Accessories->Terminal), as it will automatically gather
and attach updated debug information to this report:

apport-collect -p xorg <replace-with-bug-number>

  Second monitor Planar PL1910N 19", first I-Inc iH282 28".

  System was originally a 12.04 LTS server converted to xubuntu-desktop
  using the task-xubuntu or such.

  The second monitor uses an invalid refresh rate, even in lightdm (login 
screen).  
  I have tried 'xrandr --output VGA-0 --auto --left-of HDMI-0 --output HDMI-0 
--auto,
  however that doesn't help.
  When the screensaver is switching from off to on, after switching back to 
this computer from another (using a KVM for the second (VGA) monitor and 
keyboard and mouse, and a HDMI video switch (doesn't maintain myth of connected 
display like KVM does), the second monitor displays the screensaver (i.e. has 
right refresh rate), until the second monitor activates.

  This is not a problem with a current Debian Wheezy computer on the
  same configuration (but different ports on the switches).
