I have noticed that when the ethernet cable is unplugged, the icon does
show up, but is non functional.  All of the icons now seem to be merged
into a single menu, so when you click on the nm icon, the single unified
menu opens, but has no entries pertaining to nm.

I am guessing that this merged menu was a change in gnome-shell that
network-manager had to change to accommodate, and ubuntu updated gnome-
shell but not network-manager.
