I am terribly sorry, but there might me another issue hidden within it.
When adding an _inline_ image (making double sure that the link option
is not selected), saving and opening the resulting odt on another
machine, the image always appears to be linked and not available
locally. As this is not part of this issue all I can do is to provide
the defunct odt and the image itself in a zip.

  Export to PDF or Printing to paper/PDF with EPS images fail

Status in “libreoffice” package in Ubuntu:
  Incomplete

Bug description:
  Hi Folks,

  After upgrading to 14.04, if you want to export a document as PDF that
  contain EPS images, it'll export a blank page (EPS missing). Also the
  same behavior if you try to print to paper or print as PDS (cups-pdf).

  Tested on libreoffice:  4.2.3-rc3 and 4.2.4-rc2 from PPA
