Hi, I just downloaded your test document to see if it's the same issue I've been experiencing with Trusty Tahr and a Lexmark T520 printer, using either the Lexmark, or generic PCL and PS drivers (PPDs?).
When attempting to print your file, the result is characterised by the first page being printed but scaled as if for a page of about A1 size, exactly the behaviour that I have, but only with certain documents.
