I have gotten around this issue by having my laptop go to sleep and then plugging it in then, at which point I could wake it up and things would generally work. It seems lately though that the lock screen is just frozen upon waking up (though not always, I can't seem to notice any pattern or triggers), and therefore I usually have to restart X. What I expect to happen is to be able to plug in my secondary monitor at any point, at the lock screen, when at the desktop, or when the laptop is closed or asleep, and then just have it recgonized and work without needing to reboot or restart the display server.
