I can add more information to this. It is affecting me on a fresh Lubuntu 15.04 32-bit install as well.
Evince version: 3.14.2 The pdf that is affected is here: http://media.wizards.com/2014/downloads/dnd/5E_CHARACTERSHEETSV3.ZIP It's from: https://dnd.wizards.com/articles/features/character_sheets Unzip the file, and open "DnD_5E_CharacterSheet - Form Fillable.pdf" in Evince. In Lubuntu by default this is the default application to open it. Click in any field and enter some text. Short or long, doesn't matter. It will succeed. Press tab, enter, or click away from the field. Your mouse will still work in evince, but no keyboard input will. Keyboard works fine in any other application.
