Sorry I can't answer all the questions right now, as I'm in a bit of a rush and can't try out all the old versions of Ubuntu right now. And the bug doesn't manifest itself that often, so it's hard to pinpoint what exactly triggers it. For now I can confirm that I have experienced the bug on Thinkpad T520 too. As before, restarting the NetworkManager service fixes the issue.
